Family-Managed Home Care (FMHC), sometimes called Self-Directed Care, is an option for eligible Ontario residents who want more choice and control over how home care is arranged. Instead of having care scheduled for you through the system, FMHC may allow a client (or Substitute Decision-Maker) to manage allocated funding and use it to arrange care services that match the client's needs, routines, and preferences.

FMHC is typically coordinated through Ontario Health atHome (OHAH). A care coordinator will assess needs and explain program eligibility, required documentation, and how the care plan is structured. If approved, families manage the service delivery and may be responsible for tracking hours and reporting — so partnering with an experienced agency can make the process easier and more dependable.

What is Family-Managed Home Care?

Family-Managed Home Care (FMHC) is a self-directed home care option in Ontario coordinated through Ontario Health atHome (OHAH). If a client is approved, OHAH completes an assessment and creates a plan of service. Depending on eligibility and program criteria, the client (or Substitute Decision-Maker) may be able to manage allocated funding and arrange care in a way that better fits the client's routines, preferences, and family schedule.

FMHC can be especially helpful for families who want more consistency and control over who provides care and when support is delivered. Instead of rotating caregivers, families may be able to build a more stable care routine — often reducing stress for the client and improving comfort at home.

Families should expect some administrative responsibilities, such as tracking services and meeting any reporting requirements explained by the care coordinator.
